During hospitalization for pulmonary abscesses, a 64-year-old woman presented with chest tightness, increased heart rate, and an elevated N-terminal pro–B-type natriuretic peptide. Coronary CT angiography with cinematic rendering showed a 4.2 × 3.3 × 2.8 cm coronary artery left atrial fistula in the left atrium (Figure, Movie [online]). At coronary angiography, numerous tiny feeding arteries were identified, along with 12 draining veins into the left atrium. Percutaneous transcatheter closure with metallic coils was successfully performed.
