History: An 8-year-old African-American girl presented with headaches and vomiting precipitated by voiding and bowel movements. At clinical examination, the patient was hypertensive with a blood pressure reading of 140/94 mm Hg. Urinary and plasma free normetanephrines were elevated, measuring 13,112 nmol/d and 22 nmol/L, respectively.
